Community Health Programs

The Y offers many Community Health Programs focused on providing evidence-based support to individuals with specific health needs, including cancer recovery, diabetes prevention, fall prevention, arthritis and joint issues, Parkinson's Disease, blood pressure monitoring and weight loss.

 

Blood Pressure Self-Monitoring Program

This evidence-based program helps adults with hypertension lower and manage their blood pressure. The program focuses on regulated at-home self-monitoring of one’s blood pressure using proper measuring techniques, individualized support and nutrition education for better blood pressure management. YMCA Diabetes Prevention Program

The YMCA’s Diabetes Prevention Program provides a supportive environment for individuals at risk for type 2 diabetes to lose weight and learn how to reduce the chances of developing the disease. In a classroom setting, a trained lifestyle coach facilitates a small group of participants in learning about healthier eating, physical activity and other behavior changes. Learn More

LIVESTRONG® at the YMCA is a physical activity and well-being program designed to help adult cancer survivors achieve their holistic health goals. YMCA Weight Loss Program

The YMCA Weight Loss Program is designed for individuals who want to achieve a healthier weight by making small changes to daily behaviors and forming sustainable, healthier habits. Alongside a dedicated Y professional, participants design an action plan that will bring long-term, positive change. Learn More
